Medical information including medical history, medications, allergies, adverse events, immunisations and family history

Sensitive information is stored securely online, through the clinic management system Halaxy. Halaxy takes reasonable steps to protect your personal information from misuse, interference and loss, as well as unauthorised access, modification or disclosure. They use a number of physical, administrative, personnel and technical measures to protect your personal and sensitive information. I only offer services to people over the age of 18. I do not knowingly or intentionally collect sensitive information from children under the age of 18.

When your sensitive information is no longer needed for the purpose for which it was obtained, it will be securely destroyed after 7 years.
