  • Online Zoom consultations

  • When we speak on the phone 

  • When you send me information in an email or via social media platforms (Facebook, Instagram)

The reason why I collect this information is:

  • To provide you naturopathic services, advice and support

  • As a legal requirement for record keeping purposes

 

The sensitive information I ask you to provide for this purpose may include:

  • Identifying information including name and date of birth 

  • Medical information including medical history, medications, allergies, adverse events, immunisations and family history 

  • Medical records and pathology results

 

I am committed to securely storing and handling your sensitive information. 

  • Sensitive information is stored on a password protected computer

  • Sensitive information is stored securely online, through the clinic management system Halaxy. Halaxy takes reasonable steps to protect your personal information from misuse, interference and loss, as well as unauthorised access, modification or disclosure. They use a number of physical, administrative, personnel and technical measures to protect your personal and sensitive information. I only offer services to people over the age of 18. I do not knowingly or intentionally collect sensitive information from children under the age of 18.

When your sensitive information is no longer needed for the purpose for which it was obtained, it will be securely destroyed after 7 years.

 I am subject to the following regulations regarding the collection of personal information:

  • As a qualified naturopath, I owe you a professional duty of confidentiality. I take this responsibility very seriously and will always strive to promote your best interests and will not share your personal information with any third party unless it relates to reasons and purposes explained below

  • Patient records and personal information is kept confidential at all times and access restricted to myself the practitioner

 

You may choose not to provide your personal information. However, if you choose not to be completely honest with me, I may not be able to provide you with the services that you request.

Reasons why I may disclose your personal information include:

  • To provide you with the services you have requested

  • To liaise with other health professionals

  • To send you products that you have purchased

 

In order to do this, I may share some relevant personal information - on a strictly need to know basis - with:

  • Other treating health professionals or your doctor

  • Technical or clinical support providers 

  • Suppliers and distributers such as Metagenics, Ariya Health and Vital.ly

  • Australia Post or courier companies

 

I will also disclose your information if required by law to do so or in circumstances permitted by the Privacy Act, for example

  • To public health authorities for purposes related to: preventing or controlling disease, injury or disability to you you or another person

  • To notify or assist in notifying a family member, or another person responsible for your care, in the event of an emergency

  • Where required to do so by the law (eg subpoena, court order)

I take reasonable physical, technical and administrative safeguards to protect your personal information from misuse, interference, loss, and unauthorised access, modification and disclosure.

I manage risks to your personal information by:

  • Storing files securely with password protection

  • Ensuring that only I have access to sensitive information

  • Releasing information to service providers on a strictly need-to-know basis, and

  • Conducting regular audits of my/our security systems.

 

As mentioned above, your personal information may also be stored with a third-party provider, where it will be managed under their security policy:
